Guardians are required to report on the condition of their ward as ordered by the court or as required by court rule.
This form is used when a guardian or conservator makes an application for approval of both the annual accounting and fees.
Within 90 days after appointment, every conservator must prepare and file with the court a complete inventory of the estate of the protected person. Copies are to be mailed by the conservator.
