Guardianships/Conservatorships

• Address Information Sheet (Form No. 16:8)

  Filed with the court as a record of parties to be notified of hearings.

• Annual Accounting/Statement of Assets (Form No. 16:4)

  Each year, conservators are required to file an annual accounting/statement of assets with an attached Certificate of Proof of Possession (Cty. Ct. General Rule 49).

• Annual Report of Guardian on Condition of Ward (Form No. 16:10)

  Guardians are required to report on the condition of their ward as ordered by the court or as required by court rule.

• Application, Order and Notice for Approval of Accounting and Fees (Form No. 16:7)

  This form is used when a guardian or conservator makes an application for approval of both the annual accounting and fees.

• Inventory (Form No. 16:2)

  Within 90 days after appointment, every conservator must prepare and file with the court a complete inventory of the estate of the protected person. Copies are to be mailed by the conservator.
