Waiver of Parental Notification for Abortion (Abortion Bypass for Minors)

• Instruction Sheet (Form No. 14:6)
  Provides necessary information to file a petition in court requesting an order allowing doctor to perform an abortion without notifying parent or guardian.

  Clerks of the District Court Mailing & Email Addresses. Clerk Magistrate Mailing & Email Addresses. Lists must be provided to petitioner as part of Form No. 14:6.

• Petition for Waiver (Form No. 14:7)
  To be completed and delivered to the clerk of the court.

• Notice of Appeal (Form No. 14:8)
  Used by petitioner to appeal to the Nebraska Supreme Court when dissatisfied with the order of the trial court.
